Please complete the following form if you would like me to capture pictures of your swimmer(s) at the upcoming meets. When you complete and submit this form, you are not committing to purchase copies of the images, and I am not guaranteeing that I will get pictures of your swimmer(s) (for example, I can't help it if they breath to the opposite side of the pool during freestyle), but when you fill out this form there is a greater likelihood that I will take more pictures of your swimmer(s).
Please click here if you do not want me to take pictures of your swimmer(s). While there is a chance I may get them in the background of some group images for the Makos web site, I will try to avoid photographing your swimmer(s).

If this is the first time you are requesting photos and you aren't a Mako parent or PVS Official, there is still no charge for submitting the form, but I will require a $20.00 deposit before I give you access to your private photo gallery. I will send you an email if I get enough good photos of your swimmer, and then you can send me a check or make the deposit by credit card. If you don't like the photos in the gallery, I will refund $10.00 of your deposit.
